Space and Environment Requirements

• A quiet, well-lit, and private space for the medical examiner to conduct the physical exam.
2. Clean and Sanitized Environment:
• Ensure the area is clean to maintain proper hygiene during the exam.
3. Table or Chair:
• A sturdy chair and/or table for the examiner’s equipment and for the patient to sit comfortably.
4. Bathroom Access:
• A restroom available for urinalysis, preferably close to the exam area.
Patient Preparation
5. Personal Identification:
• A valid photo ID to confirm the driver’s identity.
6. Eyeglasses or Contact Lenses:
• If corrective lenses are required, ensure they are available for the vision test.
7. Hearing Aids (if applicable):
• Bring any hearing devices required for daily activities.
8. Medical History:
• A list of current medications and any relevant medical history (e.g., surgeries, chronic conditions).
9. Previous Medical Examiner’s Certificate (if applicable):
• Provide the current or previous DOT Medical Certificate for renewal purposes.
General Requirements
10. Electricity Access:
• A working power outlet for any medical equipment that may require charging or operation.
11. Minimal Interruptions:
• Avoid disturbances from pets, family members, or loud noises during the examination.
12. Accessibility:
• Ensure the examiner can easily enter the home and access the designated exam area without obstacles.
